Undiagnosed Hypertension More Likely in Rheumatoid Arthritis


TUESDAY, Oct. 7, 2014 (HealthDay News) -- Among patients meeting guideline-based criteria for hypertension, those with rheumatoid arthritis (RA) are less likely to receive a diagnosis of hypertension, according to a study published in the September issue of Arthritis Care & Research.

Christie M. Bartels, MD, from the University of Wisconsin School of Medicine and Public Health in Madison, and colleagues conducted a cohort study to examine whether RA is a risk factor for not receiving a diagnosis of hypertension. Data were obtained from adult patients with and without RA/inflammatory arthritis from an academic multispecialty practice. All participants were seen regularly in primary care and met the clinical guideline criteria for hypertension, but had not been diagnosed with or received treatment for hypertension.

The researchers found that 201 patients of the 14,974 with undiagnosed hypertension had RA codes. Compared with patients without RA, those with RA had equivalent primary care visits and more total visits. The likelihood of hypertension diagnosis was 36% in RA patients and 51% in patients without RA. The risk of hypertension diagnosis was significantly lower for patients with RA in adjusted Cox models (hazard ratio, 0.71), with more undiagnosed hypertension seen than with other comorbidities.

"Given heightened cardiovascular risks in RA and the importance of hypertension diagnosis as a first step toward controlling risk, rheumatologists should collaborate to improve rates of diagnosis for this modifiable cardiovascular risk factor," the authors write.

Smokers Taking Excess of Sodium Are at Higher Risk of Rheumatoid Arthritis

A new study has shows that cigarette smokers, who consume excess of sodium, are at a higher risk of developing rheumatoid arthritis.

On an average, an American's daily intake of sodium is about 3,400 mg. The Dietary Guidelines for Americans recommend limiting sodium to less than 2,300 mg a day  or 1,500 mg for those aged 51 years or older, or if they are black, or if the person has high blood pressure, diabetes or chronic kidney disease. Some people's bodies are very sensitive to sodium. This was seen in the latest study by Umea University, Sweden, researchers.

According to the Swedish researchers, there exists a strong association between high sodium intake and rheumatoid arthritis, a chronic inflammatory disorder that affects the small joints in hands and feet. The risk doubles in case of current smokers.

The study led by Bjorn Sundstrom looked at whether the recent animal and human cell study revealed the association between a high sodium intake and RA would produce similar results in a case control study that worked on data retrieved from Vasterbotten Intervention Programme (VIP).

"These findings will provide new insights into the etiopathogenic process leading to the development of RA among smokers. The finding of sodium being a risk factor for the development of RA among smokers is intriguing, as it may explain discrepancies in previous studies of diet as a risk factor for RA. That consumption of fruit and vegetables is associated with a lower risk of developing RA, while consumption of protein, red meat, and fish with a medium fat content is associated with a higher risk, could be explained by these dietary products being associated with a higher intake of sodium. These results could have implications for analyses of diet in other conditions in which inflammation is of importance," said Sundstrom.

The researchers looked at 386 individuals who reported their dietary habits as a part of the community intervention programme with a median of 7.7 years before the onset of RA symptoms. This group was then compared to 1886 controls that were taken from the same database.

As a part of the study, the researchers worked on health-screening data that included dietary habits, physical exercise, smoking and other social factors. They also collected participants' blood samples. However, the researchers did not find any link between sodium intake and the developed RA due to which they failed to confirm the hypothesis.

Sundstrom added: "Additive interaction analyses suggested that approximately half (54%) of the increased risk from smoking in the development of RA is due to interaction with sodium intake. A large influence of sodium intake on smoking as a risk factor for RA is also supported by the fact that we could not identify any significant proportion of risk from smoking in individuals with a low sodium intake."

The finding was documented in the journal Rheumatology.

Eight Ways To Look After Your Knees

Knee pain can start from an injury to even the smallest  part of your knee and can arise from overuse in childhood or adulthood.

5 Don’t ignore pain: Many people brush off knee pain when they first start to experience it and believe it will eventually go away.

Early diagnosis is key to preventing more severe and irreversible damage. If you have a sharp, intermittent, localised pain you should get checked out sooner rather than later in order to stop some conditions getting worse.

Tip: If you have a pain you can point to on your knee and it is tender to touch, make an appointment with your GP to check whether you have an unstable meniscus tear.

Tip: Unless your doctor has told you otherwise, or you are sensitive to them, try a non-steroidal anti-inflammatory tablet the night before exercise.

Rebuilding Together Provides Relief For Kearney Woman With Rheumatoid Arthritis

Jeanette Santucci of Kearney was diagnosed with rheumatoid arthritis in 2001. Rebuilding Together Clay County has helped her over the past year and made her disease more manageable.

Rebuilding Clay County recently installed a walk-in bathtub for Santucci. The tub not only makes it easier to bathe, but it has jets that Santucci said relieve the pain in her joints caused by her arthritis.

“It brings the swelling down in the joints because when the swelling is up, it hurts really bad on top of the pain that’s already there,” Santucci said. “It gets the swelling down in the joints so I can tolerate walking and tolerate being awake.”

Santucci has had numerous surgeries to replace her joints and has many more to come. She said her rheumatoid arthritis has been so unbearable that she can’t even get out of bed. Santucci received help from Rebuilding Clay County after her physical therapist suggested she contact the nonprofit to help with home repairs since Santucci’s condition prevents her from doing many things.

“I let them know what needs I needed in order to make myself livable on my own again,” Santucci said.

Several Kearney businesses and organizations helped with the installation and materials of the walk-in tub, including Kearney Rotary Club, Kearney Masonic Lodge No. 311, Kearney Lions, KCB Bank, Kearney Trust Co., Platte-Clay Electric Cooperative, Porter’s Lumber and West Kearney Winnelson Plumbing.

Santucci is thankful for all of the volunteers and groups that helped her.

“I kind of felt like the town was wrapping their arms around me and saying, ‘OK, we love you, you’re here. We’re going to take care of you,’” Santucci said.

The tub isn’t the first helping hand Rebuilding Clay County has given Santucci. Her rheumatoid arthritis prevents her from opening certain door knobs and repairing some basic items around her house. Rebuilding Clay County installed a new door handle and lock in addition to fixing her garage door.

Clay McQuerry, executive director of Rebuilding Clay County, said he was surprised at the number of Kearney businesses and organizations that stepped up to help Santucci.

“We’re always looking for organizations to partner with us, but this is really the first time that we had several organizations respond like that,” McQuerry said. “It was a welcome surprise.”

McQuerry said Rebuilding Clay County’s mission statement is to have safe and healthy homes for residents of Clay County.

“Thankfully, we have a lot of great volunteers and contractors to make that possible,” McQuerry said.

Dealing with joint pain? Try swimming

INDIANAPOLIS (WISH) – You’ve pounded your legs on the pavement for years and just can’t seem to put in the miles you used to. Try hanging up the running shoes; don a swimsuit and goggles for the workout that gives you the highest cardio-respiratory fitness compared to any other sport: swimming.

Kris Simpson, an ACSM certified personal trainer, a basketball coach and triathlete admits the swimming portion of the “swim, bike, run” was very challenging.

“Lats and back were sore, your shoulders are tired, my core got stronger, my triceps – [I have] nice swimming arms now,” says Simpson.

The challenge of swimming is in the resistance. Water offers between 12 to 14 percent more resistance than air, which forces you to build and tone muscles faster. In a study of men who completed an eight-week swimming program, they saw a 23.8 percent increase in the triceps muscle in the back of the arm.

“The fact that you’re using your arms and your legs and you’re trying to move through the water makes it a bit of work, makes it a great workout,” says Simpson.

Researchers at IU’s Counsilman Center for the Science of Swimming found that swimmers of all ages had more lean muscle and trimmer waists and hips than non-swimmers. In a separate study, they found regular swimming substantially delayed signs of aging in terms of blood pressure, muscle mass and pulmonary function.

“Swimming is a great cardiovascular sport, it definitely helps with helping your heart rate and your endurance,” says clinical exercise physiologist Regina Gallerani.

Water-based exercise can help people with chronic diseases, based on research compiled by the Centers for Disease Control. For people with arthritis and osteoarthritis, it improves use of affected joints without worsening symptoms. Swimming has also been shown to improve mood in both men and women.

“People who are having trouble moving, in general, who maybe wouldn’t want to start off running, but want to get into some kind of activity, swimming is definitely a great activity to start with,” says Gallerani.

Because 90 percent of the body is buoyant, swimming provides a low-impact exercise with great cardiovascular benefits.

“It was easier on my back and easier on my body,” says Simpson. “I’ve had some knee problems — just from pounding and running on concrete and that kind of stuff, and this just makes it a nice break, easy on my body.”

If you don’t already swim, it appears Indianapolis may be a great city to take up the sport. USA Swimming recently named Indianapolis and Carmel as 10th on list for Top Swim Cities in America. The study looked at the number of USA Swimming members, swimming facilities and swimming clubs, as well as how many swimmers qualified for high level competitions — like national championships and Olympic trials.

Absence of Specific Hormone May Contribute to Arthritis Development

A Duke University study reveals that the absence of the appetite hormone leptin can determine whether obese mice experience arthritis, no matter how heavy they are. As such, the link between osteoarthritis and obesity may be more than the wear and tear caused by extra weight. The research team from Duke University conducted the study to learn whether or not the increased body fat of obesity causes an inflammatory response in joints, an imbalance of the immune system that signals chemicals in osteoarthritis. The researchers studied mice that were leptin-deficient or deficient in leptin receptors, and both types of mice overate and gained weight.

The study mice were then compared with normal mice to document knee osteoarthritis. The measurements included pro- and anti-inflammatory cytokines present in arthritis, and several tests to assess bone changes in the knees of the mice. The results of the study showed that the knee bones of the leptin-free, obese mice did change, but without forming osteoarthritis. The levels of inflammatory cytokines, which correlate with arthritis, were largely unchanged in this group. The results suggested that leptin might have a dual role in the development of osteoarthritis by regulating both the skeletal and immune systems, according to a Duke Medicine news release.

Farshid Guilak, PhD, of Duke University, says, “We were completely surprised to find that mice that became extremely obese had no arthritis if their bodies didn’t have leptin. Although there was some earlier evidence that leptin might be involved in the arthritis disease process, we didn’t think that there would be no arthritis at all.” Guilak adds, “However, in another study, we found that mice that gained half as much weight on a high-fat diet but processed leptin normally showed significant knee osteoarthritis.”

In regards to what this means for obesity, Guilak says, “Obesity is still the number one preventable risk factor of osteoarthritis, but now it seems body fat by itself is not what is causing it.” Guilak explains, “With obesity and osteoarthritis, there are good similarities between humans and mice. If we can find a pathway that links a high-fat diet with arthritis, then we can try to identify and block the inflammatory mediators that are linked with the dietary fat.”

How Omega-3 Supplements Lessen Severity of Osteoarthritis?

Omega-3 fatty acids may improve joint health and reduce the risk and severity of arthritis, according to a study conducted by researchers from Duke University and published in the Annals of the Rheumatic Diseases on July 11.

The findings suggest that, contrary to prior belief, the types of fats that make up a person’s diet may have more influence on their arthritis risk than their weight does.

“Our results suggest that dietary factors play a more significant role than mechanical factors in the link between obesity and osteoarthritis,” senior author Farshid Guilak, PhD, said.
What You Eat, Not What You Weigh

Prior studies have established that obesity is a major risk factor for osteoarthritis, a disorder characterized by degradation of the joints and more commonly known simply as “arthritis.” Until now, scientists have assumed that a higher body weight causes increased wear-and-tear on the joints, but this hypothesis never explained why arthritis is so common in non-weight-bearing parts of the body such as the hands.

In a previous study, the Duke University researchers found that abnormally low levels of the appetite-regulating hormone leptin in obese mice were an accurate predictor of arthritis. This suggested that metabolic, rather than physical, symptoms of obesity might actually be behind arthritis.

“This made us think that maybe it’s not how much weight you gain, but what you eat,” Guilak said.

To further explore the connection between diet and arthritis, the researchers induced osteoarthritis in mice by deliberately injuring their knees. Trauma or injury is responsible for approximately 10 to 15 percent of all osteoarthritis cases. The arthritic mice were then fed either a diet high in saturated fat, a diet high in omega-6 fatty acids or a diet high in omega-6 fatty acids with a small amount of added omega-3s.

“A healthy diet would include roughly equal ratios of these fats, but we’re way off the scale in the Western diet,” Guilak said.

Health researchers now believe that the typical Western diet contains too much saturated and omega-6 fat, and not enough of omega-3 fat.
Omega-3s Slow Disease Progression, Speed Wound Healing

The researchers found no connection between body weight and arthritis. They did find that, while mice on both the saturated fat and omega-6 diets experienced a worsening of their arthritis symptoms over time, mice in the omega-6 plus omega-3 group did not.

“While omega 3 fatty acids aren’t reversing the injury, they appear to slow the progression of arthritis in this group of mice,” Guilak said. “In fact, omega 3 fatty acids eliminated the detrimental effects of obesity in obese mice.”

The researchers also noticed that mice in the omega-3 group healed faster from an injury that the researchers had inflicted on their ears (a way to differentiate between the mice) than the mice in the other two groups.

“We found that independent of body weight, dietary fatty acids regulate ear wound healing and severity of osteoarthritis following joint injury in obese mice,” lead author Chia-Lung Wu said.

The study was funded by the National Institutes of Health, the Arthritis Foundation, the North Carolina Biotechnology Center and a Taiwan GSSA graduate fellowship. It is only the latest in a glut of recent research illuminating the many health benefits of a diet high in omega-3 fatty acids.

Early Menopause Increases Risk of Heart Failure

Hitting menopause before 45 years of age could up the risk of heart failure, especially for smokers, new study suggests.
The study was conducted by researchers at the Karolinska Institute in Stockholm, Sweden. Previously, research has shown that early menopause is associated with increased risk of cardiovascular diseases. In the current study, researchers found that hitting menopause early could lead to heart failure.
Heart failure is when the heart cannot pump enough blood to meet the body's requirements. In this condition, the left ventricle - the heart's main pumping chamber, loses its efficiency.

The study was based on data from 22,000 enrolled in the Swedish National Patient Register and Sweden's Cause of Death Register, and 90,000 women in the Swedish Mammography Cohort.
Researchers found that women, who underwent menopause between 40 and 45 years of age, had a heart failure rate of 40 percent than women who hit menopause between 50 and 54 years of age. Additionally, heart failure rate dropped two percent for every one-year increase in age of menopause.
Smokers, on an average, undergo menopause a year earlier than non-smokers. Researchers found that a history of smoking was linked to heart failure risk, even in women who quit smoking. 
It is always a good idea to stop smoking. Study has shown that women who smoke have more hot flashes as they transition to menopausal stage, than other women. In white women, with certain genetic variation, smoking can lead to symptoms of menopause nine years earlier than average.

"Menopause, early or late, is always a good time to take more steps to reduce heart disease risk through exercise, a healthy diet, weight loss, and quitting smoking," said Margery Gass, NAMS Executive Director, MD, as per a news release."This thought-provoking study should encourage more research that could unveil how early menopause and heart failure are linked, and whether the factors that cause heart failure also cause ovarian failure?"
Related studies have shown that those who go through menopause early are nearly twice as likely to develop osteoporosis later in life.

'Menopause' at Clemens Center next week



Imagine four women going through menopause meeting up in a lingerie store and bonding over buying a bra while they sing and dance about hot flashes, sleepless nights and cranky moods.

That‘s the basic plot of “Menopause: The Musical,” which comes to the Clemens Center in Elmira for a two-night run starting Wednesday.

A comedy as well as a musical, the 90-minute show strives to entertain, inform and inspire the audience, Fisher added: “It’s more like a musical celebration.”

Jeanie Linders, who wrote and produced the off-Broadway show, wanted to celebrate and poke fun at an issue every woman experiences but few like to talk about, Fisher said.

“She just touched on something so relatable with a very healthy theater-going demographic,” Fisher said. “Middle-aged and older ladies like to see shows.”

“Menopause: The Musical” began back in 2001 in a tiny former perfume shop in Orlando, Fla., where less than 100 people could watch the show at any time.

“It originally didn’t get good reviews, but the audience had other ideas,” Fisher said of the show. “It just has evolved into an extremely popular show.”

From those humble beginnings, the show has become a worldwide phenomenon, with performances in 15 countries, 300 international cities and 450 U.S. cities. The show is still going strong after 13 years, said Fisher, who’s been with the show on and off since 2005.

Former employee takes Archdiocese of Ottawa to rights tribunal

Ontario's human rights tribunal will hear the case of a former employee of the Archdiocese of Ottawa who claims she was wrongly dismissed while she was struggling with both menopause and depression.

Ginette Chaumont, 59, worked in a number of positions under the Archdiocese for 22 years and was working as an administrative assistant to the Archbishop when she was dismissed on Nov. 18, 2011.

She alleges she was suffering from symptoms of menopause and depression and that these reduced her energy level and effectiveness at work.

But Chaumont alleges she did not tell her employer of her conditions at the time, though she had previously informed them in 2007 that she was suffering from depression.

She worked longer hours to keep up with her duties and did not file overtime, she alleges. But in 2011 she was dismissed her over concerns about the accuracy, efficiency and organization of her work.

'I was in a state of shock'

Chaumont alleges that after her dismissal she told her employer that her depression and menopause were the reasons for her decreased effectiveness and asked, unsuccessfully, for her job back.

"I felt abandoned, ostracized, I remember thinking I'll never be able to step into the cathedral again," said Chaumont. "I was in a state of shock."

A spokeswoman for the Archdiocese said the case is a termination of employment dispute.

"There is no foundation for a claim of discrimination on any basis. The Archdiocese is prepared to defend its position before the Human Rights Tribunal of Ontario," said spokeswoman Sarah Du Broy.

The tribunal will begin hearing the case on Wednesday.

Menopause: Nutritionists Advise Women





LAGOS - The President, Nutrition Society of Nigeria, Dr Ignatius Onimawo, has advised women approaching menopause to consult physicians and dieticians for suitable lifestyles. He gave the advice in an interview with newsmen in Lagos. Reports say that menopause refers to the time of cessation of a woman’s reproductive ability.

It means the end of monthly menstrual cycle and mostly occurs in women between ages 45 and 50.

Onimawo said that menopause was concurrent with a drop in oestrogen production which affected other hormones.

He said that the resultant hormonal imbalance manifested differently in women.

The expert listed hot flashes, weight gain, dry skin, anxiety and bone density depletion as some of the common symptoms of menopause.

Onimawo urged women in the age of menopause to take enough fruits and vegetables and maintain a balanced diet.

Another nutritionist, Miss Nike Oshinowo, who works at the Gbagada General Hospital, Lagos, advised that women should not wait for menopause to begin before adopting “beneficial eating habits”.

She urged that woman should eat soya beans, legumes and nuts - peanuts, cashew nuts and almonds - which, she said, were rich in Vitamin E.

Oshinowo said,“ Vitamin E is anti-aging.”

She also encouraged women to consume more milk which contained calcium and essential amino acids.

The nutritionist said that calcium would reduce the risk of bone diseases that occurred more as women aged.

Oshinowo said that seafood was also highly beneficial - full of beneficial fats and Omega 3.

She said that these were beneficial to the heart and could prevent breast cancer and hypertension.

She, however, advised women not to abuse the diet options, saying that taking an option in excess was not beneficial.

“For best results, people should not only eat well, they should also live well by shunning a sedentary lifestyle and engaging in exercise.

“Women should avoid loneliness,’’she added.

Also Menopace Plus does not provide money back guarantee. there are too many products available OTC that are really effective and safe since it is better to choose the right option as a menopause supplement.

Even If A Woman Has Menopausal Symptoms, It Does Not Mean That She Cannot Get Pregnant


A WOMAN is considered to have reached menopause if she has not had a period for 12 consecutive months. This is a retrospective diagnosis. Although the average age of the menopause is 50-51 years, the time at which the menopause is reached varies between women.

The menstrual periods become irregular before they stop altogether. As long as a woman has a period, the ovary will still produce some eggs (ovulate). As such, the possibility of pregnancy exists, although the chances get increasingly remote nearer to the menopause.

Even if a woman has menopausal symptoms, it does not mean that she cannot get pregnant. About 75% of pregnancies in women above the age of 40 years are unplanned. It has been reported that 93% of women aged 40 to 55 years with regular menstrual cycles appear to ovulate each cycle.

As such, although there is a natural decline in fertility after the age of 37 years, effective contraception is still necessary to prevent an unplanned pregnancy.

There are various birth control options (contraception) available. Contraceptives are either hormonal (e.g. the Pill) or non-hormonal (e.g. condoms).

Hormonal contraceptives are often featured disproportionately in the media, usually with frightening headlines. Mature women are more likely to be sensitive to these negative messages, and therefore, often abandon effective contraceptive methods.

The reality is that the contraceptive methods available are very safe. In addition, many methods have significant non-contraceptive benefits to the user.

The doctor can help a couple choose an appropriate method depending on medical history, sexual habits, the method’s effectiveness and possible side effects, lifestyle and preferences.

Contraceptive pill

Hormonal oral contraceptives (the Pill) containing one hormone (progestogen) or two hormones (oestrogen and progestogen), provide effective birth control. Fertility is restored rapidly on discontinuation.

In general, the Pill is not prescribed in pregnancy, suspected pregnancy, smokers above 35 years, or those with a history of uncontrolled high blood pressure, blood clots, coronary artery disease or breast cancer.

The low-dose combined Pill (COC) is safe and effective for perimenopausal women who have no medical conditions. It is not prescribed for women above 35 years who are smokers, or those above 40 years with cardiovascular disease like angina, or those with a history of stroke or migraine.

The progestogen-only Pill (POP) is safe for women at risk of blood clots, or who have had a heart attack or stroke. However, it is less effective than the COC in preventing pregnancy.

The added benefits of the Pill include a reduced risk of endometrial and breast cancer, with the reduced risk persisting for 15 years or more after stopping. There is also less fibrocystic changes in the breasts, better regulation of periods, and reduced postmenopausal bone loss.

There is a very slight increase in the relative risk of breast cancer in all current users of the COC. As the incidence of breast cancer increases with age, this may become more significant in those above 40 years of age.

The side effects of the Pill are few, and may include nausea, bleeding between periods, breast tenderness, fluid retention, and new or worsening headaches.

The use of the Pill may make it difficult to know when menopause is reached.

Contraceptive patch and vaginal ring

The contraceptive patch and ring are like the COC, except that the hormones are delivered continuously through a patch stuck on the skin or through a ring inserted into the vagina.

They are effective and easy to use. There is no need to consume a tablet daily. The periods are lighter, more regular and less painful. However, some users may complain of skin irritation.

The conditions for use are the same as the COC.

Intra-uterine contraceptive device

An intrauterine contraceptive device (IUCD) is inserted into the uterine cavity by the doctor. Most devices contain copper and are effective for three to five years. Fertility is rapidly restored on removal.

The IUCD string must be checked periodically to confirm that it is in place by placing a finger into the vagina. The doctor will usually arrange a pelvic examination at least once annually.

An IUCD cannot be inserted in pregnancy, suspected pregnancy, those who have a history of pelvic inflammatory disease, sexually-transmitted infections (STI), anaemia or abnormal vaginal bleeding, or those in a polygamous relationship.

The side effects may include spotting, irregular, prolonged or heavier periods, or uterine cramps. These usually occur soon after insertion, which may be uncomfortable.

An IUCD inserted before reaching the menopause has to be kept in place for one year in women aged above 50 years, or two years if the periods stop below 50 years of age.

Contraceptive injections

A single injection of a depot progestogen provides effective contraception for one to three months, depending on the brand. Fertility returns within a year after discontinuation.

It is not prescribed in pregnancy, vaginal bleeding of undetermined origin, liver disease, blood clotting conditions and breast cancer.

The side effects may include menstrual cycle changes and weight gain. Regular visits to the doctor’s clinic for the injections are required.

The conditions for use are the same as the POP.

Contraceptive implants

The progestogen implant system contains capsules, each about the size of a match, which are inserted under the skin of the inner aspect of the upper arm. It is effective and fertility is rapidly restored on removal. The implants must be removed after five years of use.

It is not prescribed in pregnancy, undetermined vaginal bleeding, liver disease, breast cancer or blood clots.

The side effects may include nausea, weight change, acne, vaginal dryness, irregular uterine bleeding and headaches, especially in the first year after insertion.

The conditions for use are the same as the POP.

Barrier methods

Barrier methods, which include the male and female condom, diaphragm, cervical caps and spermicides, must be used during every act of sexual intercourse to be effective.

The condom is the only effective method that prevents pregnancy and protects against STI. Condoms can be used in combination with other contraceptive methods.

Fertility awareness

Some women use fertility awareness methods by abstaining from sexual intercourse at certain times in the menstrual cycle. These methods cannot be depended upon if the menstrual cycle is irregular, and indeed, irregular cycles are common during the menopausal years.

Emergency contraception

Emergency contraception (EC) is effective if used within 72 to 120 hours after unprotected sexual intercourse or condom accidents. The “morning-after Pill” must be taken within 72 hours.

An IUCD can be inserted within 120 hours. They should not be used as regular birth control methods.

Mature women need not be shy or embarrassed about asking for EC.

Double protection

A woman is exposed to STIs, including HIV/AIDS, if she has more than one sexual partner, or the sexual partner has more than one sexual partner.

In such situations, there is an additional need to prevent transmission of STIs.

Whenever there is a possibility of transmission of STI, doctors will strongly recommend dual protection, either through the simultaneous use of condoms with other contraceptive methods, or through the consistent use of condoms alone for the prevention of pregnancy and the transmission of STI.

Stopping contraception

If one is using non-hormonal contraception, it should be continued for two years after the last menstrual period in women under 50 years of age, and for a year after the last menstrual period in women more than 50 years.

If one is using hormonal contraception, one cannot depend on the periods, which are hormone withdrawal bleeds, to know if one is fertile or not.

Some women on hormonal contraceptives have irregular or no periods, but are still fertile if they stop using their contraceptives.

The COC, contraceptive patch, vaginal ring and injection should be stopped at the age of 50 years and another contraceptive method used instead.

The POP, contraceptive implant, IUCD and barrier methods can be used until the age of 55 years, after which, contraception will not be needed any more. This is because no spontaneous pregnancy has been reported after that age.

Women with premature menopause because of medical conditions, e.g. undergoing chemotherapy, may have protracted fluctuating ovarian function. This would require special attention from the attending doctor.

Hormone therapy is not a contraceptive

Hormone therapy (HT) prescribed for menopausal symptoms like hot flushes, do not provide contraception, as it does not suppress ovulation due to its low levels of hormones.

Many women commence HT before they reach the menopause. It is difficult to know when the menopause occurs and how long contraception is needed in HT users.

If one has reached the menopause (had no period for one year if aged over 50 years, or for two years if under 50 years) before commencing HT, then contraception should be used until the age of 55 years.

A method of confirming if the menopause has been reached is to stop the HT medication for six to eight weeks, and then check blood hormone levels.
